23-B1   Partial articular sagittal fracture of the distal radius

23-B1 is a medial articular fracture of the distal radius with the fracture line developing in the sagittal plane.

This fracture is usually the consequence of high-energy trauma. This fracture may be seen in association with an avulsion fracture of the origin of the medial collateral ligament.

This fracture may be more frequent in sporting dogs.

Clinical signs observed for 23-B1 fractures are:

• Lameness
• Pain on manipulation
• Swelling
• Carpal instability related to the attachment of the medial collateral ligament
  
  Note: Instability is the result of either the radial fracture itself or damage to the substance of the medial collateral ligament. It is extremely unusual to see both in the same patient.

Craniocaudal and mediolateral views of the antebrachium are needed.

In the event there is no medial radial fracture, craniocaudal stressed views may be necessary to further evaluate carpal joint instability.

A CT scan may be helpful in identifying the fracture.
