Isolated injuries of the posterior cruciate ligament (PCL) in the adolescent can usually be treated conservatively.
The knee is initially stabilized in a PCL brace locked at 20°–30° or a cylinder cast for 6 weeks.
This brace provides posterior support to prevent translation. This can also be achieved with a molded cast.
Make sure that there is no posterior subluxation of the tibia after application.
Conversion to a dynamic PCL brace after 6 weeks may assist with rehabilitation with the range of motion gradually increased between 6 to 12 weeks.
