Supine, figure-of-four

The patient is placed supine on a radiolucent table.

To access the medial malleolar fracture, abduct and externally rotate the hip joint. Flex the knee 90 degrees and rest the foot on the anterior aspect of the contralateral knee, with appropriate padding.

Use blankets, or a foam pillow, to support the knee of the injured limb.
